Instructions for Registering for WIA Services

Thank you for your interest in receiving services with the Middle Georgia Consortium under the Workforce Investment Act (WIA). To begin the application process for WIA services with the Middle Georgia Consortium, Inc., please click on the "Want to Apply for WIA Services! Click Here" at the top of the page or the "MGCI Initial Assessment form link" near the bottom of this page. In order to access and print this Initial Assessment form, you must have access to Adobe Acrobat Reader and a printer. There is a free version of Adobe Acrobat Reader available at http://www.adobe.com/products/acrobat/readstep2.html.

WIA is not an entitlement program; therefore, services are not provided to everyone. Our ability to serve you depends upon the following:

You are eligible under WIA rules;
You reside in our service area - Baldwin, Crawford, Houston, Jones, Monroe, Peach, Twiggs, Pulaski, Putnam, or Wilkinson County;
You have selected training meeting our requirements;
You have the qualifications necessary to complete training and obtain a training related job; and
Funds are available to provide assistance.

Once you complete this Initial Assessment form, you may return it to the Middle Georgia Consortium, Inc. by bringing it into our office at 124 Osigian Blvd., Suite A, Warner Robins, Georgia 31088, by mail at the address indicated on the bottom of the form, or by fax at (478) 953-2509. You may also submit the Initial Assessment Form via e-mail by clicking the "Submit By E-Mail" button on the bottom of the form (if you have Adobe Standard or Adobe Pro installed on your computer). If you are unable to print the Initial Assessment form, you may call our office at 1-800-537-1933 or at (478) 953-4771 to request a form be sent to you.

Applicants interested in attending training where PELL grants and HOPE scholarships are available, will be required to complete the federal Financial Aid Form (FAFSA) before being approved for training. FAFSA information can be obtained on-line at www.fafsa.ed.gov.

Based on the information you are submitting, we will contact you to determine if we can assist you, or place your name on our waiting list for services. If you have not been contacted after 24 hours of submitting this Initial Assessment Form, please contact our office.
